Employee performance review, from evaluator to staff member to HR
Customer production

- Community Bank of Mississippi
Controls enforced
- Evaluator identity checked against the HR system before the form opens
- access stays on the bank's existing intranet permissions. Typed-name attestation by evaluator and staff member, in auditor-cleared wording. A required concur or do-not-concur mark from the staff member (a disagreement is captured, not blocked). HR approval before anything is filed, with a send-back loop that keeps prior content and clears the signature. Filing is automated only. Each supervisor sees only their own reviews.
Evidence produced
- The signed PDF filed with HR, the score and last review date written to the HR system, and Callvu's completion record (when it completed, where it was sent).
Claims
Community Bank of Mississippi runs three processes on Callvu: loan processing, lender-placed insurance exceptions and HR reviews.
At Community Bank of Mississippi, performance reviews carry typed-name attestations from both parties, pass an HR approval gate, and are filed automatically to the HR system and the HR inbox.
